If you are a subcontractor interested in providing services
to Amako, keep in mind, the following:
During the bidding process, you may be asked to provide:
- Insurance certificate showing proof of insurance
- Performance and Payment Bond letter showing ability to bond
The following
must be submitted and adhered to as in our Policies and Procedures if
a contract is awarded to your company:
- Occupation Health and Safety Program must be submitted to job site
at first visit to work at the job site
- Insurance certificate with the proper limits must be submitted before
allowed on the job site
- Drug and Alcohol policy must be signed at time of signing contract
- W-9 IRS form to be submitted to accounting prior to first payment
of pay application
- Pay application to be submitted monthly as described in procedure
document
- Lien releases must be signed and submitted monthly at time of payment
